There are a few things in VioLens MCM setup that you can try. My setup looks like below. If all you are seeing are decapitations, there are a couple of things you can do. It is merely a condition, so a null condition should effectively do nothing, so removing the null condition should fix that. Probably he intended to add a condition for it to run, and forgot it.
